RCC Transitions Committee
Committee Mission Statement
The purpose of the Transitions Committee is to identify opportunities to ensure safe and timely transitions of care for individuals in Rutland Community. Transitions include but are not limited to the movement of individuals to and from Rutland Regional Medical Center and Skilled Nursing Facilities, Assisted Care, Home Care, and Community care. The group will use collaborative techniques to identify areas which need improvement. The group members will actively engage in process improvement activities, either individually or in subgroups, to design and implement new processes.
Committee Goals for 2021
- Ensure continued identification & management of social determinants which impact safe transitions across the continuum.
- Impact access by improving availability & use of telehealth.
- Care Navigator – document care coordination for patients in the ACO.
- Standardize screening process – contributes without duplicating other subcommittee goals.
